Additional required documentation - all applicants

All applicants must arrange for the following to be sent directly to the Registrar, Citizens School of Nursing:


Each applicant is required to have official transcripts sent from the high school and all other higher education institutions where the applicant has taken courses. An official transcript is signed, bears the seal of the school, and is mailed directly to the Registrar, Citizens School of Nursing.

Test scores

The results of College Entrance Examination Board testing (SAT scores) are required to be sent directly to the Registrar, Citizens School of Nursing.


Three (3) completed Personal Reference forms (download from the link at top) are required to be sent directly to the Registrar, Citizens School of Nursing.

The 3 written references are required from mature persons not related to the applicant and are to reflect the applicant's seriousness of purpose and personal characteristics, which indicate potential to fulfill the role demands of the professional nurse. Suggested sources of references are guidance counselors, teachers, clergy, and an employer or person in a supervisory capacity. References from family members and significant others are not acceptable.

Further additional required documentation from applicants requesting transfer credit/ advanced standing

Follow instructions for those applying for transfer of college credits or advanced standing. Additional documentation may include:

  • College or LPN course descriptions or challenge test results
  • Evidence of LPN licensure in Pennsylvania
  • NLN Mobility 1, Book 1 score
  • ATI-TEAS score
  • Letter from the applicant explaining the reason for transfer (NOTE: An applicant is eligible for transfer status only if enrolled in an educational program within the last year)
  • Letter of recommendation from the director or dean of the former nursing program
