Medical Records

If you have been a patient at Saint Vincent Hospital in Erie, Pennsylvania, you can request a copy of your medical records by calling the medical records correspondence office at 814.452.5071 between 8 a.m. and 4:30 p.m. , Monday through Friday. How you intend to use your medical records determines your next step.

Requesting Records for a Saint Vincent Physician
If you are requesting records for a physician who is a member of the Saint Vincent medical staff, the Saint Vincent Hospital medical records office can send your records directly to that physician. Call the medical records office with your request and the name of the physician the records are to be sent to. No written authorization is needed.
Requesting Records for Other Physicians or Medical Facilities
If the records you are requesting are for a doctor who is not a member of the Saint Vincent medical staff, or if they are for another health care facility, they can be obtained only by written request with your authorization. After calling the medical records correspondence office to request your records, you may either mail your written request with authorization or sign an authorization form in the Medical Records department. We will mail the requested records for you at no cost. If you prefer, your medical records may be obtained from the medical records office on the first floor of  Saint Vincent Hospital. There is a charge for all records hand carried out of the Hospital.

Requesting Records for Personal Use
You can obtain a copy of your medical records for personal use. Please call the Medical Records Department at 814.452.5071.   
Non-Hospital Records
Saint Vincent can release records for Saint Vincent Hospital patients only. Our medical records personnel do not have access to medical records from any other hospital. Nonhospital records, such as physician office records, must be requested directly from the physician's office.
