When it becomes difficult to deal with your illness on your own, Healthcare at Home’s palliative care team can help.
Unlike hospice—which is end-of-life-care—palliative care aims to improve your quality of life as you battle a disease. You can receive palliative care at any stage of illness and in conjunction with all other curative treatments. We can deliver palliative care concurrently with life-prolonging care or as the main focus of care.
Working with you, your primary care physician and your family, Healthcare at Home’s palliative experts will help you:
- Plan for post-hospital care at home or an inpatient facility
- Understand your treatment choices and choose the best option
- Provide you with an individualized plan specific to your needs and symptoms
- Coordinate treatments from different caregivers
