Home Recovery Care

AHN Home Recovery Care gives you two options to safely recover with hospital-level or skilled-nursing-level care in your own home.

Home Recovery Care is a 24/7 service available for patients at Allegheny General Hospital, Forbes Hospital, and Jefferson Hospital. Call (412) 860-0685 for more information.

Make a full recovery from home sweet home

Sometimes you don’t need to be in the hospital or a skilled nursing facility when you get the right support at home. With AHN Home Recovery Care, we start your treatment right away at the hospital, then we get you back home to recover instead of keeping you there. And our Home Recovery Care Plus program gives you even more flexibility, providing in-home skilled nursing care if you need infusions or therapy after your hospital stay.

Your doctor will let you know if either option is right for you. 

How it works

You visit the hospital

While you’re here, your doctor will let you know if you’re eligible for either program. There are lots of conditions we can treat and help you recover from, right at home.

We treat you so you’re well enough to go home

We’ll get you started on the road to recovery right in the hospital before you leave.

We set up care in your home

You’ll get a computer tablet to help us track your progress, and a dedicated nurse who will visit you every day so you get all the care you need. You may even have a physical or occupational therapist visit, depending on your situation.

Benefits of home recovery care

You can be comfortable in the safety of your own home, and still get all the high-level care you need to make a full recovery, including:

  • A specially trained nurse who visits you to check on your progress.
  • A physical or occupational therapist who helps with rehab in your home for two to three weeks, if needed.
  • A recovery care coordinator to answer all your medical questions, available 24/7 by phone.
  • A computer tablet to communicate with your doctor and monitor your progress.

Conditions we treat

There are lots of medical conditions that we can treat from home, including:

  • Asthma
  • Congestive heart failure
  • Pneumonia
  • Urinary tract infection

We can also help you with rehab at home, including:

  • Home infusions
  • Wound care
  • Physical therapy
  • Occupational therapy

What to expect in the first three to five days

Our services begin when you leave the hospital. We’ll make sure you have everything you need right at home. You’ll get:

An experienced nurse, who will:

  • Visit you at home for a few hours every day.
  • Check on your progress and monitor your blood pressure, temperature, and weight.
  • Administer IV medications.
  • Help you take your medications.
  • Help you manage any pain.

A recovery care coordinator, who will:

  • Be available 24/7 to answer your questions or handle any problems that come up.
  • Make sure any equipment you need is delivered right to your home.
  • Coordinate your care with all of your doctors, including your regular doctor and any specialists you may have.
  • Monitor your progress.
  • Help you schedule follow-up appointments.
  • Provide you with health education.

Equipment to monitor your progress, including:

  • A computer tablet to communicate with your care team.
  • A blood pressure cuff, scale, or other easy-to-use devices.

What to expect the last 25 to 27 days

After the first few days, we’ll continue to monitor your progress as you start to resume your normal routine. We’ll transition your care back to your regular doctor, but your recovery care coordinator will still be available for you 24/7. We’ll help you schedule follow-up visits with your doctor, too.

With Home Recovery Care Plus, our support lasts up to 60 days

You’ll be sent home with the same monitoring equipment, visiting nurse, and support from a care coordinator for up to 60 days. And since your recovery will include rehab, you’ll have a physical or occupational therapist work with you at home every day for the first two to three weeks, too.
