About the program

The broad goals and objectives of the Surgical Critical Care Fellowship at Allegheny Health System are:

Patient care

The fellow will provide patient care that is compassionate, appropriate, and effective for the treatment of health problems and the promotion of health. The fellow will gain competence in a number of procedures, diagnostic tests, and skills that are utilized in a state-of-the art surgical intensive care unit

Medical knowledge

At the completion of the fellowship, the fellow will demonstrate knowledge of established and evolving biomedical, clinical, epidemiological and social-behavioral sciences, as well as the application of this knowledge to patient care

Practice-based learning and improvement

The fellow is expected to develop skills and habits that enable them to: systematically analyze clinical practice using quality improvement methods and implement changes with the goal of practice improvement; locate, appraise, and assimilate evidence from scientific studies related to their patients’ health problems; undertake investigations into the various areas of surgical critical care, such as new instrumentation, identification of important physiologic parameters, evaluation of pharmacologic agents in critically ill patients, or health outcomes and/or health policy issues related to surgical critical care

Interpersonal and communication skills

The fellow must be able to demonstrate interpersonal and communication skills that result in the effective exchange of information and collaboration with patients, their families, and health professionals. The fellow, upon completion of the training program, will be expected to teach the specialty of surgical critical care

Professionalism

The fellow will demonstrate a commitment to carrying out professional responsibilities and an adherence to ethical principles

Systems-based practice

The fellow will demonstrate an awareness of and responsiveness to the larger context and system of health care, as well as the ability to call effectively on other resources in the system to provide optimal health care. At the completion of the training program, the fellow will be expected to have the skills to administer a surgical critical care unit, supervise specialized personnel, establish policy and procedures for the unit, and coordinate the activities of the unit with other administrative units within the hospital

Overview

In addition to the broad, competency based goals and objectives, there are specific, unit (rotation) based goals and objectives. The majority of the fellow’s time during the year is spent in the Trauma ICU (TICU). The other required rotations serve to compliment the fellow’s experience in the TICU and to further enhance the acquisition of knowledge and technical skills. Since it is not possible for the fellow to experience every possible critical care clinical problem during the fellowship, some aspects of critical care must be covered by other means: Grand Rounds, conferences, didactic teaching sessions, board review sessions, completion of online training modules or self-directed learning
