Living with a serious illness can be challenging for patients and families. There can be fears about the future, uncontrolled symptoms, confusion about complex medical decisions, and frequent trips to the emergency room or hospital.

The AIS Home Visit Program is an innovative program committed to providing an extra layer of support for you and your family throughout the course of your illness by:

  • Helping you control pain and symptoms
  • Providing emotional support
  • Helping you make decisions regarding your care
  • Coordinating healthcare services
Members who might benefit from this program include those with advanced cancer, congestive heart failure, chronic obstructive pulmonary disease, kidney failure, advanced dementia, and many more conditions.

As part of the AIS Home Visit Program, you will have access to a care team of doctors, nurse practitioners, social workers, and other specialists who work with your existing doctors to develop a personalized plan of care including:

Symptom Management

Get expert help managing symptoms such as pain, nausea, shortness of breath, constipation, difficulty eating or sleeping, and depression. AIS Home Visit providers can prescribe medication when needed and support is available 24/7 for urgent needs.

Care Planning and Coordination

Your AIS Home Visit team will partner with your doctors to support you in your home and ensure you receive the best care possible. Working in close consultation with you, your family, and your doctors, your AIS Home Visit provider will help you set healthcare goals and do everything possible to help you achieve them.

Education and Resources

Your AIS Home Visit provider can educate you and your family about your illness, plan of care, medications, and community resources. By knowing more about your illness and plan of care, you will feel more confident in your health care and empowered to better plan for the future.
