Home Care Cardio-Pulmonary Assessment

This is a 90 minute refresher course designed for home care nurses to refine their home care skills. The primary purpose of this course is to teach home care nurses how to accurately evaluate cardiopulmonary statuses and how to assess wounds. At the conclusion of the course, participants will be expected to exhibit proficiency in medication reconciliation, patient education of red flags, telemonitor, and the usage of appropriate interdisciplinary services.

Learning Objectives

  • Perform mini-assessment to include cardiopulmonary status (vital signs, heart tones, lung sounds and edema)
  • Demonstrate medication reconciliation and teach back/show me method to ensure all medication and prescription needs are appropriate and up to date
  • Evaluate the patient for additional interdisciplinary services (PT/OT/HHA/MSW/Speech)
  • Assess heel wound to include measurements and description
  • Demonstrate patient education of red flags and telemonitor usage
  • Make MD follow-up appointments, if needed
  • Use of SBAR communication technique and documentation

Expected Outcomes

  • Accurately assess cardiopulmonary status and assess wound
  • Demonstrate proficiency in medication reconciliation, patient education of red flags, telemonitor usage, wound assessment, and the usage of appropriate interdisciplinary services
