Pilot Study: An Observation of the Provider Referral Process following the Implementation of Healthy Steps for Older Adults Workshop in a Healthcare Setting

Purpose: 

The purpose of this research study is to gain insight on how effective the referral process, as part of the HSOA program design, is on connecting participants to care and engaging them with the healthcare system. The observation of the post-referral process via past and future chart reviews will provide valuable information on when and how often participants connect with their PCP, the types of health services which are suggested, prescribed, or recommended to reduce the participants’ risk factors for falls, and the level of engagement with the healthcare system via uptake of healthcare system services.

Inclusion Criteria

Adults who are 50 years of age or older

Must be able to read and understand English and consent for themselves

Must not have previously completed a HSOA workshop hosted by AHN (may have participated in a HSOA workshop through a different agency)

Exclusion Criteria

Individuals who cannot speak, read, and/or write in English

Individuals unable to provide informed consent on their own behalf

Individuals who have previously completed a HSOA workshop hosted by AHN (may have participated in a HSOA workshop through a different agency)
