Urology specialists at AHN Surgery Institute are known for their comprehensive expertise and pioneering work in complex urological issues. Among the AHN physicians helping patients restore function and quality of life is Kiran Sury, MD, a fellowship-trained reconstructive urologist who now offers ProACTTM Adjustable Continence Therapy for Men experiencing accidental urine leakage following prostate treatment.

Expanding treatment options for post-prostate treatment incontinence

Since its inclusion in the American Urological Association guideline on incontinence after prostate treatment, Dr. Sury has incorporated ProACT into his practice for select patients with mild, moderate, and severe stress urinary incontinence. The minimally invasive procedure expands treatment options and complements existing therapies such as slings and artificial urinary sphincters.

ProACT consists of two small, silicone balloons that are placed where the prostate was removed or resected and connected with tubing to a port placed just below the skin in the scrotum. The fluid-filled balloons support and apply pressure to the bladder neck, which helps prevent accidental leakage of urine.

Careful patient selection drives successful outcomes

According to Dr. Sury, AHN urologists are focusing on treating nonradiated patients with ProACT due to current guideline recommendations.

Candidate selection begins with a comprehensive evaluation designed to distinguish stress urinary incontinence from urge-related symptoms — which can often coexist following prostate treatment. The thorough assessment includes symptom scoring, detailed clinical history, cystoscopy, and office-based screening methods to ensure patients receive treatment most appropriate for their condition and goals.

Tailoring treatment over time

Dr. Sury emphasizes a highly reliable quality care culture built on specialized training, meticulous surgical technique, thoughtful patient counseling, and ongoing follow-up.

"Every successful surgery comes down to two things: setting clear expectations with the patient and maintaining a high-reliability surgical approach," said Dr. Sury.

That philosophy is especially important with ProACT because achieving optimal results can take time. Unlike some treatment options that deliver immediate results, ProACT requires adjustments as the system is customized to patient needs.

By combining advanced training with careful maintenance, Dr. Sury works closely with patients throughout the treatment journey to help maximize outcomes and satisfaction.

Specialized expertise for complex cases

Patients with complex urologic conditions often require specialized evaluation and treatment that extends beyond routine care. At AHN, they benefit from an established urology program that includes specialized centers such as the Prostate Cancer Center, Renal Center, and Center for Men's Health and Genitourinary Reconstruction.

Providers should consider referral to Dr. Sury and urologists at AHN Surgery Institute for patients presenting with complex men's health concerns, including:

  • Persistent stress urinary incontinence after prostate treatment.
  • Urethral stricture disease requiring reconstruction.
  • Ureteral stricture disease.
  • Erectile dysfunction that has not responded to conservative therapies.
  • Complex genitourinary or abdominal reconstruction needs

"Early referral is always key," said Dr. Sury. "The earlier we evaluate patients, the more opportunities we have to identify the most appropriate treatment strategy and drive long-term outcomes."
