Medical Records
If you need to request copies of your AHN medical records, you can choose from the following options:
Request medical records through MyChart
Request medical records using the Authorization for Release of Protected Health Information form
Submit a medical record request online
Electronic Health Information (EHI) Export
With a MyChart account, you can fill out the necessary form and have your records sent to you only getting the information you need even faster. Follow these steps:
- Log in to MyChart within a web browser or MyChart app.
- Under the Your Menu tab, click Request Medical Record.
- Fill out the fields on the form page, sign electronically, and submit.
- You can view your requested records in the Document Center once they've been released.
MyChart medical record requests cannot be used to send records to any third party. To authorize the release of your medical records to a third party, you may use the online medical record request link or complete the paper form for the release of protected health information.
You can use the authorization form to have records sent to yourself or to any third-party requester.
You can also use this form to have a non-AHN provider request or send records to Allegheny Health Network.
- Download the Release of Protected Health Information form.
- Carefully fill out each section of the form. Under the “information will be released to…” section, you will either A) fill out your personal information if you want the copy of your medical records or B) fill out the doctor, medical facility or other third party where you’d like your records sent to.
- Once you’ve completed the form, you can mail it to the provider or facility, fax it in, or bring it with you to your next doctor’s appointment.
- AHN hospital information is listed on the final page of the form. If you need to find a specific provider or facility, search here.
New York state resident? If you need records from Westfield Memorial Hospital, or are looking to transfer records to the facility, you must use this form.
Requests submitted by a patient, or their representative, require a valid government-issued photo ID must be uploaded (e.g., driver’s license, state-issued ID or passport.) Expired IDs will not be accepted.
For patient representatives please provide supporting documentation in addition to your Photo ID (e.g., Medical Power of Attorney, Will, Proof of Guardianship, Letter of Representation, or Death Certificate).
To submit a request, use the following:
EHI Export produces structured machine-readable data from AHN’s Epic electronic medical record system as required under the 21st Century Cures Act.
The EHI Export will contain data that is not readable by the human eye and instead, is required to be ingested and read by a machine, such as healthcare record keeping software.
AHN is not able to provide information on how to ingest the EHI Export into a machine, nor does AHN provide any additional guidance or technical support to patients who seek to obtain and review their EHI Export.
If you are seeking an EHI Export, please note the following disclaimers
- Any HIV, Mental Health, and Drug/Alcohol treatment information contained in your records will be produced. The EHI Export module is not able to exclude data based on type of record, date range, provider, or any other exclusion criteria. By agreeing to disclose your EHI, you must agree to disclose all EHI in AHN's EPIC system, including any sensitive data.
- All requests for EHI will require completion of an authorization form. If you request to send an EHI Export to a third-party that party may be assessed a fee as permitted by 45 C.F.R. 171.302.
- Once you disclose your EHI to a third party, it may be subject to re-disclosure by the recipient and may no longer be protected by HIPAA or federal privacy law.

Cost considerations
There’s no fee to release medical records to doctors or health care facilities. However, there may be a fee if you’re asking for your records for personal use or to release records to another third party. Allowable fees are determined by state law and the Health Insurance Portability and Accountability Act (HIPAA).
