Geriatric Specialty Care

What is geriatric medicine?

As our bodies age, our health care needs change. Sometimes medications or diseases affect older adults differently than younger adults. A geriatric medicine provider helps older patients and their caregivers manage complicated health issues caused by aging.

Geriatricians are physicians who have a background in either family medicine or internal medicine and have done additional fellowship training and are board-certified in geriatric medicine, which means they have specialized expertise for treating older adults.

When should you see a geriatric medicine provider?

Generally, you should consider seeing a geriatrician for a consultation if you're 65 and older and you:

  • Want guidance on healthy aging and staying independent.
  • Are interested in a comprehensive geriatric assessment to review memory, mobility, medications, and overall health.
  • Have concerns about memory changes, falls, frailty, or physical limitations.
  • Are managing multiple medical conditions or medications that feel complex or overwhelming.
  • Are unsure if your current care, support at home, or living situation is meeting your needs.

Geriatric care at AHN: Why choose us?

AHN geriatric specialists are focused on helping you achieve your health goals and live the life you want. We focus on treating age-related conditions and managing your health so that you can continue doing the activities you enjoy. We make getting that care convenient by offering:

  • In-person or telehealth-based cognitive assessments and comprehensive geriatric assessments.
  • Specialized inpatient geriatric services at select hospital locations.
  • Home-based primary care visits (available for select homebound patients who meet eligibility criteria).
  • Oversight for rehabilitation or long-term care at designated skilled nursing facilities.

Conditions a geriatric medicine specialist can help manage

A geriatrician helps manage conditions that are directly associated with aging, including:

  • Memory changes, dementia, and Alzheimer's disease.
  • Falls, balance problems, and functional driving decline.
  • Sudden confusion (delirium).
  • Depression, anxiety, and other mood changes.
  • Weight loss, poor appetite, and nutrition concerns.
  • Caregiver stress and support for family members.

Many seniors have more than one illness and take several medications. Geriatricians work alongside your primary care provider to coordinate and help manage complex health conditions and courses of treatment. They also help to educate and support patients and their caregivers.

AHN geriatric specialists provide medical care to older adults across all settings. These services include inpatient, outpatient, skilled nursing facility, and home-based geriatric care options.
